Informed Consent for EMStrong Electromagnetic Chair Treatment
Nature of Treatment: The EMStrong chair uses focused electromagnetic energy to stimulate pelvic floor muscle contractions. During a typical 28-minute session, the treatment delivers approximately 12,000–15,000 targeted muscle contractions. You remain fully clothed throughout the procedure.
Expected Benefits: Potential benefits include improved pelvic floor muscle strength, enhanced bladder control, reduced urinary incontinence, improved core stability, and enhanced intimate wellness. Individual results may vary. A typical treatment course consists of 6–8 sessions scheduled 2–3 days apart.
Possible Risks and Side Effects: The EMStrong treatment is generally considered safe and non-invasive. Some clients may experience mild muscle soreness, tingling sensations, or temporary discomfort during or after treatment. Serious adverse effects are rare but may include muscle pain, skin irritation, or temporary changes in bladder or bowel function.
Contraindications: This treatment may not be suitable for individuals who are pregnant, have a cardiac pacemaker or defibrillator, have metal implants in the pelvic area, have had recent surgery (within 6 months), have an active pelvic or urinary infection, have a seizure disorder, or are undergoing cancer treatment in the pelvic region. Voluntary Participation: Your participation in this treatment is entirely voluntary. You have the right to ask questions about the procedure at any time. You may withdraw from treatment at any point without penalty or loss of benefits to which you are otherwise entitled.
Right to Withdraw: You may stop a treatment session at any time by informing the practitioner. You may also discontinue your treatment course at any time.
Liability Release and Waiver
By signing this waiver, I acknowledge that I have been informed of the nature, benefits, and risks of EMStrong electromagnetic chair treatment. I understand that results are not guaranteed and may vary from person to person.
I hereby release and hold harmless The Hot Spot, its owners, employees, and agents from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury that may be sustained by me as a result of, or in connection with, the EMStrong treatment.
I confirm that the information I have provided in this waiver, including my health screening responses, is accurate and complete to the best of my knowledge. I understand that withholding or providing inaccurate medical information may affect the safety and effectiveness of my treatment.
I have read this informed consent and liability waiver in its entirety. I understand its contents and agree to its terms voluntarily.
